Support planning, goal setting and safety planning is completed in collaboration with the individual, focusing on a person-centred and strengths-based model. Regular check-ins in the form of ‘Key Working’ will take place but this is not prescriptive and will be dependent on the needs of the young person as to which approach is taken and how often Key Working occurs. However, all interactions will be recorded, including at least 6 weekly reviews of a person’s personal circumstances and support requirements.

The level of support provided will be increased or decreased as required and in consultation with the placing authority to meet individual needs, especially for young people with learning difficulties and/or disabilities, as some may require additional 1:1 support.

All young people will receive a tailored support package designed with their full involvement; from a person-centered approach to promote independence. This is using information gathered from a thorough initial assessment, risk assessment and multi-agency working with other professionals and family members who can support the young person to positively achieve their aspirations.

Young people will receive support in the following areas:

  • Practical and general household maintenance
  • Hygiene routines
  • Maintaining a clean and safe living environment
  • Food preparation, Cooking and Healthy Eating
  • Food and grocery shopping
  • Budgeting and Money Management
  • Joining and attending leisure and health activities
  • Accessing and registering with health services including safe sexual health
  • Self-motivation, esteem, and confidence building, including positive self-image
  • Identify and encourage the use of local facilities, community engagement and participation
  • Advocacy and assistance with appointments including transportation where required
  • Support for certain activities, support with legal issues, support with claiming benefits & move-on support
  • Support for young offenders with low level offences
  • Role Modelling – Coaching and mentoring healthy relationships, conflict resolution and positive risk-taking
  • Mentors for residents
  • Improve family relationships, social support networks as well as staying safe
  • Daily Occupation – routes back into education, employment and training
  • Daily structure and routine planning
  • Managing emergencies
  • Obtaining independent accommodation and accessing a tenancy
  • Joining the housing register and support with bidding for council properties
  • Practise healthy mental health routines and looking after your emotional wellbeing
