Support Packages

Standard Support

31.5hrs per week / Ratio 1:3

Phoenix House is staffed 24 hours per day. Each young person will be assigned a Key Worker who will work alongside them, providing encouragement and direction, and empowering the young person to move forward and reach their full potential; whatever their chosen goals are. This is achieved through our standard support package of 31.5 support hours each week for each individual resident.

All young people will receive a tailored support package designed with their full involvement; from a person-centred approach to promote independence. This is using information gathered from a thorough initial assessment, risk assessment and multi-agency working with other professionals and family members who can support the young person to positively achieve their aspirations.

1:1 Support

In addition, if it is considered that any young person will require additional support to supplement the standard support hours provided, or would benefit from initial 1-1 support, this can be discussed as part of the young person’s assessment as a supplementary cost depending upon the needs of the individual.

Resettlement / Move on Support

If appropriate, Phoenix Independence can offer Resettlement/move on package to assist with the transitional period for our residents moving into lower support or independent.

This is a bespoke package created between the Young Person, Phoenix, the Leaving Care team, and the new housing providers. It is very much focused upon their individual needs, often this package of support is based around their emotional wellbeing, but can also be tailored to assist them with practical things such as physically moving to their new accommodation, unpacking, finding local shops, setting up benefit claims etc.

Hours and duration of resettlement package vary and are adapted and priced accordingly.
